Joomla make user profile visible at login

At login I would like to see the user profile checkbox so that they can agree to the usage terms. The login form works perfectly but there is no checkbox for this, only the “remember me” checkbox.
Searched the forums for Login Checkbox, User Profile but couldn’t find an answer.
User Profile, is enabled and public with link to Terms of Service Article which I want a user to agree to before they can login.
But the user profile doesn’t show. Be grateful if someone would tell me what I am doing wrong. I can’t believe that it is a bug so must be my lack of knowledge.

Thanks for the help but I get this message
"pauld2, you do not have permission to access this page."
I found that if I switch to a "standard" Joomla template then the optional fields appear perfectly. So that is my only option at the moment but thanks for replying any way.
