Image Intro showing broken icon

For some articles I dont need an Intro Image, so I let my "Intro Image" field empty... then I get some broken image icons showing like this :

Firefox is correct, but Chrome and Explorer show "broken image icon", how can I get rid of this ?

(Not sure if this is a Joomla 2.5 problem or a yougrid/template problem)

MySQL Version: 5.x

dilemtl, you do not have permission to access this page. This could be due to one of several reasons:

To fix it, go to templates/template_name/html/com_content/featured/default_item.php

change to this:

Same thing in templates/template_name/html/com_content/category/blog_item.php
